Good Faith Estimate Notice

For clients who are uninsured or choose not to use insurance.

Your right to an estimate

This notice explains the right to receive a Good Faith Estimate. It is not the individualized estimate itself. Christian Counseling NFP must prepare and deliver a separate estimate containing expected services and charges when federal requirements apply.

If you do not have insurance or do not plan to use insurance to pay for care, you usually have the right to receive a written Good Faith Estimate of expected charges when you request one or schedule services far enough in advance. Ask Christian Counseling NFP for an estimate before scheduling if one has not been provided.

What the estimate includes

The estimate should identify the expected services and charges known at the time. It is not a contract and may not include unexpected services, changes in clinical need, services from other providers, or services scheduled separately. Tell Christian Counseling NFP if the planned frequency, service type, clinician, or payment status changes so the estimate can be reviewed.

If your bill is higher

If the final bill from a provider is at least $400 more than that provider’s Good Faith Estimate, you may be able to use the federal patient-provider dispute-resolution process. Keep a copy of your estimate and bills. Information is available at www.cms.gov/nosurprises (opens in a new tab) or by calling 1-800-985-3059.
